2. Health IT tools related to data analysis and care coordination — which are particularly important for managing chronic conditions — have led to an increase in the adoption of solutions in the population health management market.

3. Healthcare providers will account for the largest share of end-users in the population health management market by 2025. In 2016, providers held 40 percent of the market share.

4. Major players in the market include Cerner, Philips, Lumeris, Health Catalyst and McKesson, among others.
