The breach, which occurred between Sept. 5 and Sept. 7, may have compromised the data of current and former patients of the Watertown, S.D.-based health system and former Glacial Lakes Orthopaedics patients.
Information such as names, addresses, dates of birth, medical record numbers, provider or facility name, conditions, diagnosis and/or treatment information, payment information, and dates of service may have been compromised.
According to Prairie Lakes, there has been no evidence to indicate that any of the information has been misused.
On Dec. 15, AAA Collection sent letters to all affected Prairie Lake and Glacial Lakes patients.
