The error affected letters intended to alert patients and their guardians between May 17 and July 25 about changes to their care plans. However, as a result of the error, a second address line not associate with the intended clients was printed on the mailing labels — meaning some clients may have received information that did not belong to them.
The error was corrected July 25.
The letters were intended for people with developmental disabilities, and may have exposed clients’ names, address, case numbers, Medicaid client ID numbers, plan of care numbers, explanation of benefits, providers’ names and authorized services.
“As a result of this computer error, DDS clients may have received information not belonging to them, their information may have been mistakenly sent to another person, or both possibilities may have occurred,” the agency said in a statement, according to newsok. “It is also possible that no information was affected by the computer error. However, out of an abundance of caution we are notifying all persons who received a Plan of Care change notice between these dates of the possibility their information was compromised.”
