  1. Morley Companies (Saginaw, Mich.): 521,046 individuals affected
  2. South Shore Hospital (Chicago): 115,670 individuals affected
  3. Comprehensive Health Services (Cape Canaveral, Fla.): 106,752 individuals affected
  4. AccelHealth (Brownwood, Texas): 48,126 individuals affected
  5. Jacksonville (Fla.) Spine Center: 38,000 individuals affected
