Patients affected by the data breach received a letter letting them know what information may have been compromised. The state of New Jersey was first notified about the breach on June 30.
Hackers obtained a limited amount of information that was being held by an outside company, according to NJ.com.
A call center has been set up for patients to determine if they were affected.
