The CEOs of four major for-profit health systems earned a combined $90.5 million in total compensation in 2025, with bonus structures behind those packages that provided an inside look at the distinct philosophies of what performance looks like at the top.
Tenet Healthcare (Dallas): Saum Sutaria, MD, received $43,108,969 in total 2025 compensation, the largest package of the four. His 2025 annual incentive bonus was funded at 200% of target, with the committee tying performance to adjusted EBITDA and free cash flow, according to a proxy statement filed April 16 with the Securities and Exchange Commission. Dr. Sutaria’s annual incentive plan payout was $9 million, three times his $3 million target.
HCA Healthcare (Nashville, Tenn.): Sam Hazen received $26,456,606 in total compensation in 2025, according to a March 13 proxy statement filed with the SEC. Mr. Hazen’s annual incentive was weighted 80% on EBITDA and 20% on quality metrics. The quality portion covers healthcare-associated infections and sepsis, complications, and mortality and care experience. His target bonus was 175% of base salary.
Universal Health Services (King of Prussia, Pa.): Marc Miller received $16,148,937 in total 2025 compensation. Mr. Miller’s target incentive bonus reward was 150% of base salary, calculated based on adjusted net income per diluted share and return on capital, with payouts ranging from 0% to 200% of target, according to a proxy statement filed with the SEC.
Community Health Systems (Franklin, Tenn.): Kevin Hammons, who became permanent CEO on Oct. 1, 2025, received $4,772,869, the smallest of the four and reflecting his partial-year tenure and CHS’ tighter financial position. Mr. Hammons received 119% of his target cash incentive based on full-year 2025 performance, according to a proxy statement filed April 2 with the SEC. CHS layers financial targets with a non-financial performance component and an overachievement tier on top.
