The accountable care partnership is effective June 1. It will focus on proactive services, chronic disease management and care navigation.
“We’re pleased to partner with UnitedHealthcare to improve both the quality and value of care patients receive when they see a WKCC care provider,” John Rubino, MD, WKCC chairman of the board said in a statement. “Together, we can continue to improve the patient experience while making an impact on the health of our community.”
WKCC, which was created by Raleigh-based WakeMed Health & Hospitals, WakeMed Physician Practices and Key Physicians, has more than 400 physicians and has participated in the Medicare Shared Savings Program since January 2014. This partnership is one of 250 new accountable care initiatives launched by UnitedHealthcare this year, and one of the payers more than 520 active programs.
