PinnacleHealth and Capital BlueCross have had an accountable care arrangement since 2012, in which the insurer provides nursing resources, access to technology and critical analytics to network providers. Additionally, the two organizations implemented a patient-centered medical home pilot in 2010.

“Our partnership with Capital BlueCross is an extension of our patient-centered medical home for high risk and rising risk patients while our commitment to providing accountable care emphasizes the consistent achievement of high quality with costs that are below the region’s average,” Michael Young, FACHE, president and CEO of PinnacleHealth, said in the news release.

“It is incumbent upon insurers and healthcare providers to find innovative ways to help employers achieve workforce health goals and lower costs,” Gary D. St. Hilaire, president and CEO of Capital BlueCross, said. “CareConnect offers this service and PinnacleHealth’s reputation as a trusted provider makes this plan really attractive to businesses seeking the best possible care for their employees.”

For patients, the insurance product will have lower out-of-pocket costs and an incentive program that provides rewards for health and wellness activities. They can also visit any provider in the PinnacleHealth network without a referral.
