Michigan physician network forms joint venture with value-based care platform

Agilon Health is partnering with a West Michigan physician network to boost value-based care outcomes, particularly among seniors.

Agilon is entering the 20-year joint venture agreement that will see it offer its value-based care platform to Holland Physician Hospital Organization, a network of over 50 affiliated community physicians that collaborates with Holland Hospital.

“This partnership with Holland PHO highlights the desire of primary care physicians to transition to a model of care for their senior patients that ensures quality and improves outcomes,” Steve Sell, CEO of Agilon Health, said in a statement.

Agilon health works with over 2,700 primary care providers operating in approximately 30 communities around the country. The company reported $690 million in revenue for the fourth quarter 2022 and revenue of $2.71 billion for fiscal 2022.
