Somerville, Mass.-based Mass General Brigham is improving access to care outside of traditional clinical settings with its Community Health Improvement Corps.
The Corps is a workforce of community health workers with clinical training who are supported by a registered nurse who assists with triage and care coordination, according to an Aug. 3 system news release.
They will deliver services like screenings, navigation to care management, health-focused social risk screenings, and education for colorectal cancer and hypertension in locations, such as libraries, housing complexes, and places of worship.
The program launched in four communities and plans to expand to seven more communities later this year.
