Inova Discharge Planning Clinics opened a location at Inova Loudon Hospital to facilitate the transition from the hospital to the community for patients with chronic diseases, such as diabetes, chronic obstructive pulmonary disease/pneumonia, congestive heart failure and asthma.
The clinic will provide primary medical care, medical case management, medication reconciliation, social work interventions and transition to medical home. The program aims to stabilize complex patients and prevent hospital readmissions.
Inova Juniper Program, a provider of HIV/AIDS care, also opened a location at Inova Loudon Hospital. The clinic provides primary medical care, diagnostic and lab services, pregnancy management, medication assistance, nutrition counseling, mental health therapy, substance abuse counseling and support groups.
Both clinics encompass a total of 1,875 square feet in a building on the Loudoun campus, and house four exam rooms and a therapy room.
