How Lancaster General Health Manages High-Risk Patients

Lancaster (Pa.) General Health has developed a new care model, Care Connections, to help high-risk patients with high rates of utilization better manage their care.

Through the program, Lancaster General Health opened a Care Connections clinic in Lancaster General Hospital in August 2013. The clinic has a team of pharmacists, care management nurses, social workers, navigators and behavioral health professionals who work as a team to assist patients.

“About three percent of the population drives 50 percent of healthcare costs,” John Wood, MD, medical director of Care Connections, said in a news release. “The goal of Care Connections is to provide better care at the right time in the right place, with the hope of reducing the cost of care by 15 percent.”

There are 48 patients currently enrolled in the Care Connection program. Three patients have already graduated from the program, after “successful attainment of their goals with the Care Connections team and the primary care providers” who will care for them regularly, according to the news release.

Care Connections is not the only step Lancaster General Health has taken to provide accountable care: It joined the Medicare Shared Savings Program as an accountable care organization this year as Lancaster General Health Community.
