A health center employee inappropriately gained access to patient records that included names, addresses and in some cases, Social Security numbers. The employee resigned in December after an internal investigation. The investigation found privacy breaches dating back to June 2010.
The 1,400 patients whose records were compromised will be notified by mail and will receive instructions on protection against identity theft. To assist with any possible identity theft related to the situation, the health center is offering patients insurance coverage and a free, two-year subscription to a credit monitoring service.
