CCNC is the first organization to win the inaugural Hearst Health Prize, which recognizes programs, projects or interventions that measurably improve care through technology, care delivery, clinical processes or care guidance information.
CCNC earned the prize by developing a transitional care management model that helped reduce hospitalization and readmission rates for the target population by 10 percent and 16 percent, respectively, in the last eight years. The program also lowered total Medicaid costs by 9 percent.
