Continuum is a physician enablement company providing practice management, population health and community care services to provider groups. Continuum will add Caradigm to its integrated health platform, combining Continuum’s existing best-of-breed technologies, care management best practices and expertise with Caradigm’s population health solutions to scale its managed care services business as it assumes as increasing number of value-based contracts.

According to the press release, Continuum will implement Caradigm Care Management, Quality Improvement and Risk Management solutions as well as the Caradigm Intelligence Platform (CIP) and Knowledge Hub.
