Aledade’s New Jersey ACO to manage care for 20k+ Horizon BCBS members

Aledade, a value-based care network operating ACOs across 18 states, will coordinate care for Horizon Blue Cross Blue Shield of New Jersey’s commercially insured and Medicare Advantage members.

Under the agreement, physicians in Aledade’s New Jersey ACO will be held accountable for improving the health outcomes of their Horizon patients. Physicians will be rewarded for reaching certain quality and cost-containment targets. More than 20,000 Horizon members are expected to participate in the collaboration.

In August 2017, Aledade revealed plans to form a primary care physician-led ACO in New Jersey in January 2018.
