A Vizient survey of 168 hospitals across 39 states found that poor coordination — not bed shortages or staffing constraints — is the primary driver of excess length of stay, challenging the assumptions behind most length-of-stay reduction programs.
Just 2% of admissions account for 15% of prolonged hospital days, costing health systems more than $2 billion annually at roughly $2,093 per excess day, according to Vizient data and a utilization study that spanned from 2001 to 2012.
Hospital occupancy, which is already strained, is also on track to exceed the 85% safety threshold by 2032.
Vizient’s survey paired hospital responses with LOS index data to identify which strategies correlate most strongly with better outcomes. Four nonclinical coordination capabilities emerged as the highest-impact levers: standardize escalation of discharge barriers, identify and intervene early for vulnerable high-risk patients, include social determinants of health support in care delivery rather than outsourcing it, and case manager-led interdisciplinary rounds.
Execution gaps are significant. Only 36% of hospitals have implemented systemwide escalation standards across more than half of inpatient units, according to Vizient’s May 12 report. Just 33% of academic medical centers report full adoption of case manager-led rounds.
And while 82% of hospitals refer patients to community resources for social needs, only 18% of academic medical centers and 28% of nonacademic hospitals report high implementation of direct support, such as housing assistance or financial aid.
The report’s core finding is that hospitals don’t lack effort; they lack aligned execution on processes that can change discharge times and free up clinical space.
