Dubbed Aledade West Virginia Health Center ACO, this new organization was launched in partnership with the West Virginia Primary Care Association. It will cover 13,000 patients and is the state’s only ACO led by FQHCs.

“Federally Qualified Health Centers play an important role in caring for medically underserved communities. Aledade is proud to partner with health centers in West Virginia to improve patient health by delivering quality, personalized and coordinated care,” Farzad Mostashari, MD, CEO and co-founder of Aledade, said in a statement.

Aledade is a company that helps independent physicians and non-hospital-based providers launch ACOs. It already runs another ACO in the state that coordinates care, manages population health and provides preventive health services for roughly 8,000 patients. Since its launch in 2014, Aledade has formed partnerships with more than 110 practices in 11 states — a footprint that covers more than 100,000 patients.
