The new ACO collaboration expands on the partnership Anthem and UH launched in December for patients enrolled in Anthem’s commercial plans. The new ACO will focus on delivering high-quality, patient-centered, preventive care to Anthem MA members through enhanced care coordination and chronic disease management.

The launch of this ACO marks UH’s 10th accountable care program. The system’s ACOs serve more than 270,000 Medicare and Medicaid beneficiaries.
