St. Vincent’s Health Partners, Anthem BCBS Sign Value-Based Agreement

St. Vincent’s Health Partners, a Bridgeport, Conn.-based physician hospital organization developed between St. Vincent’s Medical Center and regional physicians, has announced a value-based reimbursement agreement with Anthem Blue Cross and Blue Shield.

The two parties have agreed on quality metrics for hypertension, diabetes, tobacco use management and obesity identification, as well as other primary care-based metrics, to support population health management. Anthem will pay SVHP for care coordination work, and SVHP can share in the savings achieved by the new care delivery model.
