This ACO collaboration will include resources and providers from Seton Healthcare Family, Austin Regional Clinic, Lone Star Circle of Care and community physicians. Providers in the ACO will receive monthly patient updates to better monitor patient care. The providers will also keep all clinical information about their patients in a single electronic medical record.

The providers can receive payment incentives based on improvements in certain measures, such as readmission rates, disease management and prevention, patient safety and care delivery.
