The collaborative accountable care initiative with Cigna covers about 6,500 members. Cigna will pay Hunterdon Healthcare Partners for medical and care coordination services, and the PHO can be rewarded through a pay-for-value structure if it meets quality and cost targets. THe CAC initiative launched July 1.
The ACO with Horizon BCBSNJ will cover more than 18,000 lives. In the program, Horizon BCBSNJ will provide Hunterdon Healthcare Partners’ physician practices with funding for population care coordinators — nurses who coordinate treatment and engage patients in their care. Additionally, all patient treatment will be tied to evidence-based standards, and the physician practices will have increased accountability for improving quality and reducing cost.
