Humana, Unity Healthcare launch value-based care for MA enrollees in Indiana

Lafayette, Ind.-based Unity Healthcare and Louisville, Ky.-based Humana announced Thursday an agreement effective July 1 to bring value-based care to 4,000 Medicare Advantage beneficiaries in North Central Indiana.

The agreement includes more personalized care for Humana Medicare Advantage beneficiaries, as well as a range of population management tools and access to chronic care, disease management and wellness programs. The program incentivizes providers based on quality outcomes and care coordination measures.

"We're working with Unity Healthcare to put primary care physicians back at the center of the healthcare system, and give them the data and technology they need to create health," Larry Costello, Humana Senior Products President in Indiana, said in a statement.
