The new arrangement, which puts increased emphasis on health outcomes, preventive care and the patient experience, will be undertaken by LG Health Community Care Collaborative, LG Health’s accountable care organization that launched in 2014. The agreement includes three hospitals, 300 physicians and more than 40 offices.

The partnership includes established quality and cost goals for the care of the designated MA population based on standards from the National Committee for Quality Assurance Healthcare Effectiveness Data and Information Set.
