The value-based care program is similar to an accountable care organization in that it aims to provide more coordinated, personalized care for patients by tying value-based incentives to performance improvement and quality outcomes.

ConcertoHealth has a highly integrated healthcare network and several patient-centered medical homes. Under the agreement, these services will be complemented with Humana’s population health capabilities to help further improve care for Humana Medicare Advantage members.
