Horizon BCBS, Hackensack Partner for ACO

Horizon Blue Cross Blue Shield in New Jersey and Hackensack-Physician Hospital Alliance, Hackensack (N.J) University Medical Center’s accountable care organization, have announced the launch of a new collaborative accountable care agreement.

Under this new agreement, Horizon BCBS will provide care coordination payments to the ACO in exchange for the affiliated practices taking on additional accountability for improving the patient experience as well as controlling the cost of care for Horizon BCBS patients. The ACO and its affiliated practices will have an opportunity to share in the resulting financial savings.

More than 450 primary care physicians practicing at approximately 50 practice locations are participating in this newly formed ACO, which will cover more than 16,000 people, according to the news release.

“This ACO reflects Horizon’s continuing commitment to collaborate with providers to deliver better quality care at lower costs,” said Jim Albano, vice president of network management at Horizon BCBS, in the news release.
