Healthcare Solutions Network is an ACO that formed in 2014 from a partnership between Cincinnati-based health systems TriHealth and St. Elizabeth Healthcare.
Here are five things to know about Healthcare Solutions Network.
1. Though new to the MSSP in 2017, based on first quarter membership reports, Healthcare Solutions Network is among the top 25 ACOs in terms of membership.
2. In its inaugural 2016 contract, HSN achieved nearly 90 percent of its measures and earned shared savings by keeping medical costs under target.
3. Nearly all of its 300 primary care physicians are Patient Centered Medical Home Level 3 certified.
4. HSN serves patients in Ohio, Kentucky and Indiana.
5. Patients in the HSN network have access to 10 hospitals and over 1,600 providers.
