Cigna, Lahey partner for collaborative care in Boston

Cigna and Beverly, Mass.-based Lahey Clinical Performance Network launched a collaborative care initiative Oct. 1 in the Boston area for more than 7,000 Cigna beneficiaries.

LCPN is Burlington, Mass.-based Lahey Health's accountable care organization, and has been part of the Medicare Shared Savings Program since January 2013. Its new collaborative care partnership with Cigna will achieve the same population health goals of an ACO — to improve health outcomes, affordability and the patient experience.

"We established Lahey Clinical Performance Network to provide highly-coordinated, cost-effective care that would improve the health of our patients and deliver a superior patient experience," Greg Bazylewicz, MD, senior vice president, chief population health officer and president of LCPN, said in a statement. "We look forward to collaborating with Cigna as we strive to better serve our patients."

As part of the agreement with Cigna, more than 850 LCPN physicians will monitor and coordinate patient care with the help of care coordinators. The care coordinators will use data from Cigna to identify patients who need screenings or missed prescription refills, for example. They also will aid patients in scheduling appointments and signing up for Cigna's disease management programs.

Clinical and medical economics teams from Cigna will help LCPN providers hit quality and financial targets. If providers successfully hit targets, they have the opportunity to earn rewards through the partnership.
