The program is part of Cigna Collaborative Care, a value-based delivery model that aims to achieve many of the same goals as an accountable care organization. The program works to close gaps in care, increase preventive health visits, improve physician follow up and enhance care coordination.
The program will benefit more than 12,000 Cigna health plan members. It is effective July 1.
