Anthem Blue Cross and MemorialCare Medical Foundation, the physician division of MemorialCare Health System in Fountain Valley, Calif., have partnered to form an accountable care organization focused on providing coordinated treatment for Orange County, Calif. patients with chronic diseases.

Under the new agreement, Anthem PPO members will receive care that is organized by teams of physicians, registered nurses and other practitioners who coordinate medical services based on patients’ needs and also help guide patients through the healthcare system.

Anthem will provide additional compensation to MemorialCare Medical Foundation if patient costs drop while quality measures of patient care are met through the ACO.

“The traditional fee-for-service model is paying for volume: The more things you do as a physician, the more you get paid,” said Mark Schafer, MD, CEO of MemorialCare Medical Foundation, in the news release. “As in this model, we’re moving toward paying for value.”
