Aledade will collaborate with Trenton, N.J.-based New Jersey Academy of Family Physicians for the ACO, and will continue to add practices. Under the partnership, Aledade will provide NJ physicians software, data and resources to aid providers’ transition to value-based reimbursement.

“Aledade believes primary care physicians are uniquely positioned to lead the shift to and succeed in the new value-based healthcare system,” Farzad Mostashari, MD, CEO and co-founder of Aledade, said.
