The hospital said it followed the manufacturer’s instructions for disinfecting, but the manufacturer’s instructions were not correct. The endoscopes were fully cleaned with an enzymatic detergent, but a final disinfectant soak did not include one small channel in the scope. The risk to patients is low, according to the report.
The patients affected underwent the procedure between June 11, 2007 and May 28, 2010.
