Calumet Medical Center (Chilton, Wis.). Calumet Medical Center — part of Menasha, Wis.-based Affinity Health — is roughly 40 miles south of Green Bay and is the only hospital in Wisconsin’s Calumet County. The medical center is both a 2015 HealthStrong Top Hospital and a Top 100 Critical Access Hospital, as designated by iVantage Health Analytics. Calumet Medical Center also earned a five-star rating on its HCAHPS survey.
