SERPA-ACO (Crete, Neb.). SERPA-ACO is a Track 1 Medicare Shared Savings Program participant with 16 clinics across Nebraska. The clinics established a limited liability corporation to assure high quality programs involving medication reconciliation, fall prevention assessments and depression screening. SERPA-ACO operates under a patient-centered medical home mission, which emphasizes close ties between patients and physicians well-versed in patients’ particular needs.
