Optum ACO (Phoenix). Independent physicians across Arizona’s Maricopa County partnered to form Optum ACO. CMS selected Optum ACO to participate in the Next Generation ACO Model in 2016. Optum ACO distributes 20 percent of shared savings to primary care participants and reinvests 80 percent in infrastructure.
