Montefiore ACO (Bronx, N.Y.). Montefiore ACO was established in 2012 and transitioned to the Next Generation ACO Model in 2017. Montefiore’s 11 hospitals, primary and specialty care sites, community-based physicians and providers from other healthcare organizations in the region participate in the ACO.
