Essentia Health (Duluth, Minn.). Essentia Health, which serves patients in Minnesota, Wisconsin, North Dakota and Idaho, launched an ACO and joined the Medicare Shared Savings Program in 2012. The ACO earned Level 3 accreditation from the National Committee for Quality Assurance and participates in Track 3of the MSSP in 2017. Essentia teamed up with payer Medica to launch an ACO called Essentia ChoiceCare with Medica in 2016.
