Stephanie Forbes, RN. Vice President of Population Health and CHA Clinics at Community Hospital of Anaconda (Mont.). Ms. Forbes is responsible for all aspects of health IT at Community Hospital of Anaconda as vice president of population health and CHA clinics. During her tenure, she helped implement Epic EMR and used her executive leadership to enhance meaningful use reporting. Under Ms. Forbes’ leadership, Community Hospital of Anaconda attained stage 1 meaningful use in 2011 and CHA’s primary care clinics achieved patient-centered medical home designation.
