Park Nicollet Health Services (St. Louis Park, Minn.). Park Nicollet became one of the first of 32 Pioneer ACOs in 2012, but only after taking part in the five-year Physician Group Practice Demonstration, CMS’ first Medicare pay-for-performance initiative for physicians. At the beginning of 2016, Park Nicollet transitioned to become one of CMS’ 21 Next Generation ACOs. Data from 2014, the most recent year available, shows Park Nicollet generated $1.8 million in shared savings and served more than 13,000 beneficiaries.
