Project ACHIEVE seeks 300 more hospitals to complete survey on improving care transitions

In an effort to combat the lack of support of continuity and coordination during care transitions, the Patient-Centered Outcomes Research Institute and University of Kentucky in Lexington are seeking hospital survey respondents.

The "Achieving Patient-Centered Care and Optimized Health in Care Transitions by Evaluating the Value of Evidence" project, or Project ACHIEVE, uses a variety of qualitative and quantitative methods — including site visits, surveys and clinical and claims data from caregivers and providers — to identify the most important transitional care services and outcomes for patients. For its "Hospital Transitional Care Effort Adoption" survey, the partnership is seeking clinicians or staff in administration, case management and quality improvement from 300 hospitals who are willing to complete the survey and broaden the dataset.
