Max Bayard, MD, practices in St. Albans, Vt. According to a notification on his website, his offices were broken into Aug. 5. Various devices were stolen, containing patient names, Social Security numbers and other treatment-related information. The announcement indicates the type of information varies on the devices stolen, but could include birth dates, Medicare/Medicaid enrollment information, treatment dates, types of treatment and diagnoses.
A HIPAA Journal report indicates approximately 2,000 patients’ information was contained on the devices.
The practice does not believe any information has been misused.
