Washington, D.C., and 16 states are running their own exchanges. The federal government will facilitate exchanges in the remaining states.

Premium rates for state-based exchanges in California, Maryland and New York have ended up lower than expected, according to the report. HHS issued a report last month that found the lowest-cost silver plan premium in the individual market in eleven states that have released premium information is 18 percent lower than Congressional Budget Office estimates. The exchanges will open for enrollment Oct. 1.
