The partnership’s plan, called the Carelink-Duke Medical Home, is built around the patient-centered medical home model, and physicians will use electronic medical records to coordinate care for the plan’s members.
Carelink-Duke Medical Home will be powered by Duke’s network of primary care and specialty physicians, but members will also have the option to choose certain other community providers. Coventry will administer, underwrite and provide customer support for the plan.
The plan will be offered to employers in seven North Carolina counties as a way to get high-quality care at a lower cost.
