Severity Levels of the Top 5 Reported Safety Events in 2016

The top 5 safety events reported in 2016 run the full spectrum of severity categories , from Category A – circumstances or events that have the capacity to cause error, to Category I – the error may have contributed to or resulted in the patient’s death. Of the top 5 reported events, the majority (41.9%) were classified as Category C – an error occurred and reached the patient but did not cause harm. Categories E-H are those errors that caused temporary or permanent harm, and 13.34% of the top events reported in 2016 fell within those categories.
