The Centers for Medicare & Medicaid Services (CMS) is investing heavily in modernizing rural healthcare through its Rural Health Transformation Program (RHTP), a $50 billion initiative established under President Trump’s Working Families Tax Cuts legislation. The program, announced in 2025, aims to fund solutions that strengthen healthcare, improve efficiency, and address healthcare-delivery challenges in rural communities.
These are critical goals given that so many rural healthcare providers are leaving medicine entirely due to burnout and physician deserts are growing.
An October 2025 study published in the Annals of Internal Medicine, which tracked more than 712,000 physicians caring for Medicare patients, found attrition rates climbing across every specialty. Of particular concern for rural care: Female physicians and those practicing in rural areas were more likely to leave clinical practice. Replacing a physician is expensive, often costing a practice two to three times the annual salary of the physician who left, according to the American Medical Association (AMA). The Commonwealth Fund notes that 43 million people who live in rural areas face primary-care clinician shortages.
For these reasons and so many others, healthcare organizations need real solutions that stop the bleeding, so to speak. It’s an area where health tech could make a powerful impact.
One solution that stands out for its proven ability to impact rural healthcare is ambient AI technology, which leverages microphones, sensors, and machine learning technologies to capture, transcribe, and analyze clinical interactions. These capabilities make a measurable difference in rural health organizations’ effectiveness in addressing population-health challenges head on, reducing clinical burnout, and improving cashflow—all of which are aligned with RHTP goals.
Improving Clinical Workflow and Morale
While no single health IT product or service can fully eliminate clinical burnout, which impacts patient outcomes, ambient AI scribes can significantly reduce it.
In a recent study published by JAMA Network, 263 physicians and advance-practice practitioners across six healthcare systems found that after 30 days with an ambient AI scribe, burnout among those working in ambulatory clinics decreased significantly from 51.9% to 38.8%. There were also significant improvements in the cognitive task load, time spent documenting after hours, focused attention on patients, and urgent access to care.
Hawse Health, a Federally Qualified Health Center (FQHC) serving a diverse rural community in West Virginia with family and internal medicine and psychiatry, can relate to these data. Until recently, rural internet connectivity issues were a huge pain point for the organization, causing interruptions in documentation and delayed note entry. But after discovering and implementing an ambient AI scribe in 2024, providers now save several hours per day in administrative work.
“Two hours have been saved each day, I’m not staying after [at the] clinic to wrap things up,” says Tiffany Garner, assistant medical director and nurse practitioner for Hawse Health.
Laticia Hollingsworth, a physician assistant at San Luis Valley Health in Alamosa, Colo., said her organization, which serves communities across southern Colorado and northern New Mexico, also saw a huge reduction in burnout after its clinicians started using an ambient AI scribe in 2024. Previously, clinicians relied on a combination of typing and dictation tools.
“We’re a rural community, so access matters,” says Hollingsworth. “The less time we spend documenting, the more patients we can see and the more care we can provide.”
Streamlining Operations and Improving Cash Flow
As organizations like MaineGeneral Health have noted, the benefits of ambient AI extend beyond clinician satisfaction. The non‑profit health system serving Maine’s Kennebec Valley also relies on its ambient AI scribe to improve cash flow and revenue cycle management by addressing inefficiencies in clinical documentation that hamper medical billing.
“The benefits to ambient AI are immediate, with a reduction in documentation time and more focus on patients during visits, but there is a bigger, long-term potential benefit as well,” says Mark St. John, CIO for MaineGeneral Health. “We see ambient AI as a tool that can significantly reduce the administrative burden for physicians while also improving our clinical data and billing.”
These are important benefits to consider, given that the Health Resources and Services Administration (HRSA) projects that by 2037, only 68% of the demand for rural primary care physicians will be met, compared with 73% nationally.
Rural healthcare organizations will need all the help they can get to adequately care for the patients who depend on them, now and in the future.
