Virginia Hospital Center retools its discharge, post-acute transitions practices

Virginia Hospital Center in Arlington has announced plans to implement new tools to improve the efficiency of hospital staff, lower patient lengths of stay and boost compliance with the CMS IMPACT Act and its new rules on discharge planning.

An update made to the CMS IMPACT Act in 2015 requires the standardization of patient assessment data used in post-acute care settings to improve quality of care. In response to these changes, Virginia Hospital Center has collaborated with Aidin, a home health, case management and discharge planning company, to retool its discharge practices.

Using Aidin, Virginia Hospital Center will design a marketplace patients can use to select competing post-acute providers based on quality and satisfaction.

According to Virginia Hospital Center CMO Jeff DiLisi, MD, the partnership will “bring a new level of transparency for post-acute care operations and outcomes, increasing value for our patients.”
