1. Ochsner Health sent more than 100 heart failure patients home with a Carematix Blipcare wireless scale, with daily weights sent to Ochsner’s Epic EHR system.
2. Clinicians monitored patient weight and adjusted their medications accordingly.
3. The program resulted in higher patient engagement and reduced readmissions. Detailed information on the program will be presented during the 2014 American Heart Association Scientific Sessions Nov. 18.
