Transitional Care Improve Some Outcomes for Heart Attack, Stroke Patients

A meta-analysis of published research suggests hospital-based transitional care can improve some outcomes for stroke or heart attack patients, according to a study published in the Annals of Internal Medicine.

For this study, reviewers analyzed 44 studies on transitional care for either stroke or heart attack patients. The studies measured outcomes from four interventions: hospital-initiated transitional care, patient and family education, community-based support and chronic disease management.

The analysis showed moderate strength of evidence that hospital-initiated support reduced length of stay for stroke patients. Low strength of evidence showed hospital-initiated support also reduced mortality for heart attack patients. The researchers also noted there was not sufficient evidence to demonstrate benefits from the other three interventions.
