Top 3 Medical Errors at Indiana Hospitals in 2011

Stage 3 or 4 pressure ulcers acquired after admission was the most reported event by Indiana hospitals in 2011, according to the “Indiana Medical Error Report.”

The report for 2011 includes data on reportable events in hospitals, ambulatory surgery centers, abortion clinics and birthing centers. In 2011, 149 hospitals were included in the report with a total of 94 events, meaning 32.2 percent of hospitals reported at least one event. In 2010, hospitals reported 102 events.

While retention of a foreign object in a patient after surgery was the second most reported event by Indiana hospitals in 2011, the number dropped by half, from 30 events to 15, compared with 2010 numbers.

Here are the top three events reported by Indiana hospitals for 2011:

1.    Stage 3 or 4 pressure ulcers acquired after admission — 41 events
2.    Surgery performed on wrong body part — 15 events; Retention of a foreign object in a patient after surgery — 15
3.    Death or serious disability associated with a fall — 12
