Data sources for the study include MEDLINE, Cochrane Library, Cumulative Index to Nursing and Allied Health Literature, ClinicalTrials.gov and World Health Organization International Clinical Trials Registry Platform. Reviewers independently selected randomized, controlled trials reporting a readmission or mortality rate within six months of an index hospitalization. In total, 47 trials were included.
At 30 days, a high-intensity home-visiting program reduced all-cause hospital readmission rates for heart failure patients. Over three to six months, home-visiting programs and multidisciplinary heart failure clinic interventions reduced readmission rates and produced a mortality benefit.
“These interventions should receive the greatest consideration by systems or providers seeking to implement transitional care interventions for persons with heart failure,” the study authors wrote.
